Use this form to submit a report after you have
received and tested IC parts fabricated by MOSIS.
Please include at least the following information:
number of parts received, number tested, number that
were functional, and comments.

Reporting test results to MOSIS is mandatory for designs fabricated
under the
MOSIS Educational Program. Reports are optional for other
designs. However, the feedback received from all MOSIS customers is
valuable because it is used for quality control and vendor
evaluation.
