Use this form to cancel fabrication of a design in the MOSIS fabrication queue. This form cancels a previously requested fabrication, but retains the 5-digit MOSIS Design Number for future use. If you want to cancel fabrication and you do not plan to resubmit your design, you can use the Cancel Project Form to completely cancel a project.
