Use this form to completely cancel a MOSIS IC project and the 5-digit Design Number assigned by MOSIS. Once a project has been cancelled, you can no longer do anything with it except request its status. For more detailed information, see the Cancel Project Documentation.

CAUTION: If you just want to submit a revised version of your layout, do NOT use this form. Use the Cancel Fabrication form instead.
