Who Can Sign Documents for University Accounts

Customer Agreement
The two copies of the MOSIS customer agreement you send to MOSIS must each contain the original signature of a person authorized to sign legal agreements on behalf of your organization. For a university, this is usually a department chair, president, or corporate attorney, but MOSIS accepts the signature of anyone who has been given the appropriate authority.

A professor can be accepted as a signatory only upon receipt by MOSIS of a statement from the university legal department on university letterhead acknowledging the authority of said professor to enter into a legal agreement on behalf of the university.

MEP Instructional Account Application
MEP Instructional Account Application requires information on the account's primary contact. The primary contact must be the class professor or instructor. A student may not be the primary contact for an account. The application requires the signature of the primary contact, and the signature of the department chair.

MEP Research Account Application
MEP Research Account Application requires information on the account's primary contact. The primary contact must be the class professor or instructor. A student may not be the primary contact for an account. The application requires the signature of the primary contact, and the signature of the department chair.

Professor and department chair sign where indicated.

Class Enrollment Confirmation Form

AMI Semiconductor Online Access Request Form
The request must be made by the class professor or instructor. A student or other university staff member may not make the requst.

AMI Semiconductor DKLA and CDA
A professor can be accepted as a signatory only upon receipt by MOSIS of a statement from the university legal department on university letterhead acknowledging the authority of said professor to enter into a legal agreement on behalf of the university.

Individual NDA
Each individual who has access to the documents must sign an Individual NDA. The original is kept by account liaison.

Account Liaison NDA
Professor's signature may be witnessed by an other professor, or the department chairperson.
